Although it seemed as though this problem was fixed in the newest MTV3D65.dll, the problem has resurfaced. There is plenty of information and a wealth of screenshots available here:

But, to summarize the problems:

1) Some vertices are instantly translated to 0,0,0 as soon as any morphing is applied to an actor.
2) Some vertices are locked in place, and do not morph at all, causing nasty seams.

One curious detail about this bug is that the problems usually only happen on one side of the actor, despite the fact that the actor's geometry is symmetrically mirrored and symmetrically weighted to a symmetrically mirrored skeleton. The asymmetrical presence of distortions/frozen vertices makes this bug somewhat different from the earlier bug.

Redoing my skeleton removed the problem. Although I'm still not 100% sure what actions or mistakes caused the problem, I'm now not sure whether it's an issue with TV3D, the exporters, or just Max being screwy.
